CT ACQUIRED BRAIN INJURY PROGRAMS

The ABI program provides various non-medical services to individuals with acquired brain injury who meet the Department of Social Services’ criteria for program eligibility.  The program allows individuals to live outside of institutions and nursing facilities by allowing for services to be provided in their home and community settings. 

CT DEPARTMENT OF DEVELOPMENTAL PROGRAMS

The DDS program is responsible for the planning, development and administration of services to persons with intellectual disabilities and individuals medically diagnosed as having Prader-Willi Syndrome.  Services include residential placement and in-home supports, day and employment programs, early intervention, family support, respite, case management and other services.  In addition, the Autism Division operates a program for adults with autism spectrum disorder who do not have intellectual disabilities.

CT HOME CARE PROGRAM FOR ELDERS

The CHCPE program provides assistance to individuals over the age of 65 who meet the Department of Social Services’ criteria for program eligibility.  The program allows for assistance with hands-on activities and tasks such as bathing, dressing, eating, transfers and other personal care.  Individuals on the program who choose self-directed services make their own choices regarding the people they will hire to provide the assistance they need to live in their home and community. 

CT MONEY FOLLOWS THE PERSON PROGRAMS

The MFP program is a nationally featured demonstration program for rebalancing Connecticut’s long-term care system.  Individuals under the program are afforded the opportunity to have maximum independence and freedom of choice about where they live and receive services.

There is a broad array of services available under the program.  Transition Coordinators provide one-to-one assistance with coordinating the move from facility to community.  There is also assistance with directing community supports, housing, accessibility, and coordination to ensure a smooth transition.  Six different service and support options are available to meet the individual needs of those who transition to the community. 

CONNECTICUT BUREAU OF REHABILITATION PROGRAMS

The BRS program assists persons with significant physical or mental disabilities to prepare for, find or keep a job. If you are eligible for BRS services, a counselor will help you develop an employment plan based on your needs and abilities and will help arrange whatever services are needed to achieve your employment goal. 

CT PERSONAL CARE PROGRAMS

The PCA program provides assistance to individuals who have permanent, severe and chronic physical disabilities who meet the Department of Social Services’ criteria for program eligibility.  The program allows for assistance with hands-on activities and tasks such as bathing, dressing, eating, transfers and other personal care.
